Patient’s Advantage members will experience a coordination of care effort unlike anything else in the marketplace.
It all starts with the primary doctors:
- Tier 1 providers will have the ability to access Patient’s Advantage member claims information to direct care to highest quality providers in the most efficient manner possible.
- The added layer of transparency will result in the reduction of unnecessary tests and services driving down overall plan utilization and reducing cost.
- Occurring simultaneously, American Health Data Institute nurses will supplement the coordination of care efforts by doing telephonic outreach and coaching to ensure the Patient’s Advantage member has all the resources necessary to effect change when required.
- The joint effort between the Tier 1 providers and AHDI is one-of-a-kind — provider and care manager working collaboratively to improve the health of the community served.
